It's probably more noticeable with large resolutions, I have a 1440x900 display.
Did some more testing, I'm now running 0.31.12 in windowed mode.
When I first start, its clickable horizontal 78 x 23 vertical
I resize to fill my available screen, then its 86 x 47
Maximize, and its 88 x 52.
Fullscreen, and its 78 x 23 again. Probably 80x25 minus 2 grids
Changing WINDOWEDX and WINDOWEDY does not change anything.
Changing GRAPHICS_WINDOWEDX and GRAPHICS_WINDOWEDY to 1440x900 changes screen to fill my display, but clickable area is still 78x23 and I can't resize to get extra clickable tiles.
Starting at 640x480 and maximizing window results in the same 88 x 52 tiles as before.

When GRAPHICS:NO, I start at 640x300, so scaling up to 1440x900 is 2.25* x 3*
Then I get 178 x 70 tiles clickable when maximized in windows mode. That's where the 147 comes from in my bug report. |

Also, not an issue in 40d16, so I doubt its my setup |

DF 31.23 with pre-installed Phoebus graphics pack, Windows XP SP3, and I am getting this behavior at all zoom levels. At the most zoomed-in view, in windowed mode, I can mouse-designate 23 tiles from the top edge and the entire view left-right. In fullscreen mode, vertical range doubles to 46 or 47 tiles from the top and entire view left-right. This increased range persists when changing back to windowed mode from fullscreen mode.
I have to zoom out a few steps to encounter the right-edge limit, which appears to be 79 tiles from the left edge. At maximum zoom-out, the designation area is approximately one third of the display left-right and approximately one fifth top-down, from the top left corner.

Reminder sent to: Baughn Shishimaru's comment from 0005841:0022454: "Looking at the memory addresses that contain the relative position of the mouse you can see that the maximum value accepted for the coordinates x and y of the mouse pointer does not change when changing the zoom level." |

This happens on OS X Lion (10.7.5) as well. For me it usually happens when I maximize the window during the intro movies, but resizing it later during the game usually fixes the problem (even resizing it about 2 px in a direction and then back to the old size fixes it).

On XP its also linked with loading screen resizing. Do not resize the window before fortress is loaded or get only limited area responding to mouse click designation, about the original size of loading window. |

This is still an issue in 40.01. |
