Does this happen when assigning the nicknames from the in-game interface, or does it only happen when you use external utilities such as Dwarf Therapist? |

Dwarf Therapist. I will test with in-game adding but I will have to check how it is done with the original DF "interface". |

Select the dwarf using 'v' (or 'u' then 'c'), then press 'z' for status, then 'y' for customisation, and finally 'n' for custom nickname. |

I reproduced with naming using utility and later renaming. I will try without any DT use. |

No I made very small test without results (I stopped playing DF due to inability to move military without attacking every single enemy so further tests are extremely unlikely) |
