0004305: Equipment mismatch error message means nothing.
The message in the logs is: <dwarf name> cancels Pickup Equipment: Equipment mismatch.

There is no mention of what the dwarf wants and what the dwarf is expecting to get. This would REALLY help us in figuring out what is actually wrong. Being able to zoom to the spot of the offending equipment would also help.
Assign equipment to dwarves and wait. I don't know how to reproduce specifically because the message is so generic.

Are you getting this message from marksdwarves? If so, it's probably 0004084.

The ambiguity of the message is annoying, but not a bug -- you can suggest improvements here: http://www.bay12games.com/forum/index.php?board=5.0 [^]

Mostly marksdwarves. I don't use archery ranges though. I have seen axedwarves do this when they are trying to get their gear together too.

Under what circumstances does it happen? Please upload a save demonstrating the problem to http://dffd.wimbli.com/ [^]

I've seen this one a lot too, so I sympathize with you. For me it happens to some members of the military, and I've tried to isolate it. I think the problem might be due to one of these things:

1. Miners and woodworkers are problematic. I've watched them go from "train" to "pick up equipment" to cancel job, mismatch, back to "train". They already have a "weapon" that they need to use as a civilian. Yet they want to equip all the equipment you've told them to equip, including another weapon. They go to pick up the weapon and then realize they are not on active duty, and can't drop what they have. For me this results in them not equipping any equipment at all, and sticking with civilian pick/ax.
2. The equipment you're telling them to wear (for their feet socks and high boots, for instance) is not being equipped properly. For instance a dwarf may have two socks on one foot which makes it impossible for her to wear the high boot on that foot. I haven't seen this, but others have.
3. You've equipped them with a weapon that doesn't work with the barracks you've assigned them to train at(For example, they have maces and you've told them to train at the archery stand).
4.You've assigned ammo of the wrong type (bolts/arrows) or assigned ammo to melee fighters.

Do any of these match with your experience?

Also, is it possible that the "pickup equipment" is only happening on shift change, such that a dwarf that goes through his or her bolts will not get a "pickup equipment" task, and just sits around with "cannot follow orders"?

No save, and probably covered by another report.
