I can confirm this bug, it has occured several times in the museum III community game. Here is a save game:

Two characters with this issue can be retrieved in legends mode:
Arcturus Cinderfang (a player character)
Etur Equallashed (an NPC)
Both died from old age twice. The cause seems to be the following: If a character dies during world progression his death is not properly stored. For clarity, With world progression I mean when he dies not in an active fortress or when an adventurer is present, but elsewhere in the world, during processing in the background. The death will be recorded in legends mode, but when you load the site a copy of the character will be generated as if he never died. That character can die again, and a second death will be recorded.
Old age is not the only death cause that will trigger this. You can recreate the effect on a recently deceased character in this game, Bralbaard Hammerfishes. His death is not properly recorded in legends because the game was created with hidden historic events, but an export of the world-history.txt file shows him as recently deceased:

You can revive him by reclaiming the fortress "Falsetower the Citadel of Worlds" he will be one of the citizens. (It is the last fortress in the list when reclaiming, there are 30+ other fortresses, it may also be usefull to know that world gen in this world stopped in the year 700, so there's 158 years of active playing now, which may be part of the cause) |
